The national mail-order competition (mail-order diabetic testing supplies) CBA includes all ZIP codes in all parts of the United States, including the 50 states, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, Guam, and American Samoa.
Tips for Searching in Microsoft Excel:
To look up ZIP codes for a particular CBA(s):
1) Click on the arrow at the top of the CBA Name column.
2) Click on the checkbox next to Select All.
3) Click on the checkbox next to the name of each CBA you would like to view
(you may check more than one).
4) Click OK.
To look up ZIP codes for a particular state(s):
1) Click on the arrow at the top of the State Code column.
2) Click on the checkbox next to Select All.
3) Click on the checkbox next to the name of each state you would like to view
(you may check more than one).
4) Click OK.
To look up a particular ZIP code(s):
1) Click on the arrow at the top of the ZIP Code column.
2) Click on the checkbox next to Select All.
3) Click on the checkbox next to the name of each ZIP code you would like to view
(you may check more than one)
4) Click OK.
To view the full list of CBAs:
1) Click on the arrow at the top of all three columns and, if not already checked, place
a checkmark in the box next to Select All.
2) Click OK.
